You can pay for your office visits and any treatments by cash, check, credit or debit card at the time of treatment. We also accept most major dental insurances and the Citi Health Card. To discuss your payment options or insurance coverage, please call (574)772-3666.

For your convenience, we accept assignment of dental benefits and will submit insurance claims on your behalf.  We request that the patient portion is paid at the time of service.
